چکیده

71 AT Outcomes of Pediatric OSA—Huang et al INTRODUCTION Obstructive sleep apnea (OSA) syndrome is a highly prevalent condition in children and characterized by snoring, witnessed apnea, unrefreshing sleep, and excessive daytime sleepiness.1,2 Children with OSA experience recurrent periods of elevated upper airway resistance during sleep due to partial or complete upper airway obstruction, which results in snoring, episodic oxyhemoglobin desaturation, hypercapnia, and repeated arousals.3,4 The respiratory disturbance of recurrent hypoxia-reoxygenation episodes during the night is associated with an increased risk of suboptimal growth, poor sleep quality, neurocognitive dysfunction, behavioral problems, overweight status, and cardiovascular disease in childhood.5-8 The prevalence of OSA is approximately 2-3% in children,9,10 and current studies have evaluated the influence of OSA on various associated morbidities5-8,11 and tried to identify the factors predicting poor treatment outcome.12 TREATMENT OUTCOMES OF ADENOTONSILLECTOMY FOR CHILDREN WITH OSA

Rationale for and design of the "POSTA" study: Evaluation of neurocognitive outcomes after immediate adenotonsillectomy compared to watchful waiting in preschool children

BACKGROUND IQ deficits are linked to even mild obstructive sleep apnoea (OSA) in children. Although OSA is commonly first diagnosed in the pre-school age group, a randomised trial is still needed to assess IQ outcomes after adenotonsillectomy in the pre-school age-group. This randomised control trial (RCT) will primarily determine whether adenotonsillectomy improves IQ compared to no adenotonsi...

The multiple challenges of obstructive sleep apnea in children: morbidity and treatment.

PURPOSE OF REVIEW To delineate some of the major morbid phenotypes that have emerged in pediatric obstructive sleep apnea (OSA), address new concepts in our understanding of OSA-associated morbidities, and elaborate on innovative therapeutic schemes that may improve outcomes for this condition. In addition, the conceptual framework whereby a childhood condition such as OSA can be linked to spec...

Childhood obstructive sleep apnoea: to treat or not to treat, that is the question.

Childhood obstructive sleep apnoea (OSA) is relatively common, occurring in at least 2% of children with other studies estimating a much higher prevalence. Part of the reason for the controversy regarding the prevalence of OSA is the lack of a standardised definition. In children with OSA, clinical symptoms during wakefulness tend to be vague and non-specific (eg, behavioural issues) and are of...

Adenotonsillotomy Versus Adenotonsillectomy in Pediatric Obstructive Sleep Apnea: An RCT.

BACKGROUND Adenotonsillectomy (ATE) is a well-established and effective treatment of pediatric obstructive sleep apnea (OSA). In recent years, a more conservative method, adenotonsillotomy (ATT), has gained popularity because it is associated with less postoperative morbidity. Yet no previous randomized study has compared these 2 methods regarding their effectiveness in treating pediatric OSA i...

Evaluation of Children with Chronic Rhinosinusitis after Adenotonsillectomy

Introduction: Chronic rhinosinusitis (CRS), defined as an inflammatory process involving the paranasal sinuses that continues for at least three months, is a major cause of morbidity in the pediatric population and a difficult entity to treat with a poorly defined pathophysiology. The cornerstone of treatment for children with CRS remains aggressive antibiotic therapy, but many patients fail t...
